I confirm by accepting this declaration that I agree that the data controller and data processors mentioned below collect and process personal information and data about me when I accept this form as a potential or existing client or contact of Area9 Lyceum ApS (Denmark), Area9 Lyceum Inc., (Delaware, US), Area9 Learning ApS (Denmark), Area9 Learning Inc. (Delaware, US), Area9 Inc. (Delaware, US), Area9 Innovation ApS (Denmark), Area9 Innovation Inc. (Delaware, US), Area9 Technologies ApS (Denmark), Area9 Labs ApS (Denmark) (hereafter referred to as ‘Area9’).

The typical personal data that Area9 collects and processes are name, e-mail address, phone number, if any. Sensitive data are not collected or processed. The purpose of processing these data is managing customer relations, communicate, and market adaptive learning products.

I am aware that the Personal Data Act (GDPR) gives me the following rights:

• Right to be informed about the collection / processing of personal data for electronic data processing (sections 28 and 29).

• Right to ask for insight into the personal data processed electronically (§ 31).

• Right to object to the personal data being processed electronically (§ 35).

• Right to require correction, deletion or blocking of personal data that is incorrect, misleading or similarly treated in violation of the law (section 37).

I am aware that I can read more about the Personal Data Act at www.datatilsynet.dk. Area9 Lyceum ApS, Galionsvej 37, 1437 Copenhagen K, Denmark is the data controller. Area9 Lyceum ApS’ data processors are Area9 and HubSpot.com (CRM system). Data processor agreements are made with the relevant processors and the data processors are applying to the EU-U.S. and Swiss-U.S. Privacy Shield Frameworks or applying to other appropriate legal mechanisms to safeguard the transfer. Area9 does not share any personal data with 3rd parties besides the data processors. The recipients of the personal data have the same confidentiality as Area9. I confirm by my acceptance on this statement that: I agree that the listed data controller record and process personal data about me in connection with the processing, implementation, control, evaluation and impact measurement of the project. I am informed that I may appeal against the processing of personal data at Data Supervision, Borgergade 28, 1300 Copenhagen K, tel: +45 33 19 32 00, or by e-mail: dt@datatilsynet.dk.
